Personal Profile for Reading Mentors, Coaches and Staff

As part of our effort to build a culture that is personal and that celebrates education and setting and achieving goals, we will be lining our front hallway with a picture and brief description of all of the students, volunteers and staff at the SAY Detroit Play Center. Please complete the following information to help inspire our students in their own education and goals.
  • We will have three things under your name that capture your education, background and/or interests. We would like to start with your high school to reinforce for young people the many different things that people can do once they get their high school diploma.

  • We would also like to include two other aspects of your education and life that best capture who you are. Please complete at least two of the questions below:

    As a requirement for membership, our young people must set and achieve goals each semester. To help model the way, we are asking staff and volunteers to do the same. Could you please list one to three personal or professional goals that you would like to accomplish by December 31, 2015? We are trying to help our students set SMART goals, so please help us by making your goals Specific, Measurable, Ambitious, Realistic, and Time-bound.

    By December 31, 2015, I will:
    *Work out at least three times per week in 10 out of 12 weeks.
    *Prepare an outline of five sermons to be delivered in the following year.
    *Read five books related to our mission and work.
    *Create and implement an evaluation system to help achieve a culture of excellence at the center.
    *Save enough money to complete my Christmas shopping without putting anything on a credit card.
